二、申请人的资格要求：

normal style="margin-left:0.0000pt;text-indent:28.0000pt;mso-char-indent-count:2.0000;" >2.落实政府采购政策需满足的资格要求：促进中小企业发展、促进残疾人就业、支持监狱企业发展。根据《政府采购促进中小企业发展暂行办法》、《财政部、司法部关于政府采购支持监狱企业发展有关问题的通知》、《关于促进残疾人就业政府采购政策的通知》的规定，经与采购人协商确定，对符合上述要求的供应商价格给予6％的扣除，用扣除后的价格参与评审（评标时提供相应的证明材料）。但本项目属于非专门面向中小企业采购的项目。

normal style="margin-left:0.0000pt;text-indent:28.0000pt;mso-char-indent-count:2.0000;" >3.本项目的特定资格要求：3.1 具有独立法人资格的相关产品的生产厂家或针对本项目唯一授权的代理商；3.2 同一品牌的生产厂家只能授权一个代理商参加投标，如果生产厂家参加投标则不再允许代理商参加投标；3.3 需具备有效的企业资质证书：如投标人为生产厂家，应提供与投标产品一致的《中华人民共和国医疗器械注册证》、《医疗器械生产许可证》；如投标人为经销商或代理商，应提供《医疗器械经营许可证》、Ⅲ类《医疗器械经营许可证》和Ⅱ类医疗器械经营备案凭证，所投设备的生产厂家须具备与投标产品一致的《中华人民共和国医疗器械注册证》、《医疗器械生产许可证》，代理商应具有制造商（投标产品为国产品牌时）或国内总代理商（投标产品为进口品牌时）出具的授权书（国产品牌产品投标生产厂家和经销商均需提供证书扫描件并加盖生产厂家公章；进口品牌产品经销商投标提供证书扫描件并加盖国内总代理商公章）；3.4单位负责人为同一人或者存在控股、管理关系的不同单位，不得参加参加同一合同项下的项目投标； 3.5投标人未被列入“信用中国”网站（www.creditchina.gov.cn）失信被执行人、企业经营异常名录、重大税收违法案件当事人名单和中国政府采购网（www.ccgp.gov.cn）政府采购严重违法失信行为记录名单；3.6本次招标不接受联合体投标。
